Enter “Lactation Loop” as the practice name and press “sign on”. Yes, our headquarters are in Lakewood Ranch, FL. Do not add your e-mail or a password yet. Simply enter the practice name and click “sign on”. Fill out all areas that apply for insurance and mom/baby health history. Please provide a credit card as well for denied insurances and travel fees; you will be notified and invoiced after the visit. Any questions about the chart, text our main line at 941-254-2502.
If you have Aetna, Anthem, BCBS PPO plans, BCBS TX, Cigna, Curative, Meritain, UMR, UHC, Tricare Select we will bill the carrier directly. Please upload your insurance cards into the chart inside Mobile Lactation Consultant. Insurance verification will be sent via e-mail from lactationloopbilling@gmail.com. Please check your spam!

HMO Referral Policy - Patients enrolled in an HMO plan are typically required to obtain a referral from their primary care provider (PCP) prior to services being billed to insurance. Requesting the referral directly for mom and baby through your PCP, OBGYN and pediatrician results in the fastest approval. Please note: Services rendered prior to referral may be billed directly to the patient. Self Pay - We are always happy to see you at a self pay rate, so you can see your provider of choice! We do have group packages and will offer a sliding scale for those families who need a financial discount. We also have IBCLC students that are very qualified, and can see you for a low cost of $25 or free if needed. Example insurances in this situation are some HMO plans, PSO plans, community health insurances, Sunshine health, Medicaid and more.
We accept many forms of payment; credit cards, Zelle, cash, check, Venmo, Affirm, FSA and HSA (health spending accounts). We will provide you with a superbill and invoice after you pay for the visit. The superbill will be submitted by you to your insurance carrier. They should provide reimbursement according to the affordable care act laws. The amount varies based on individual contracts. We will do our best to help you navigate this. If you call your carrier and they state "Lactation is covered at 100%" but we have denied you after you have attempted to verify, it simply means we are not in network with your provider.
